Request for Proposal
The Sam Schmidt Paralysis Foundation (SSPF) seeks to support research that will document the service activities and time associated with provision of wheeled mobility and seating equipment by rehabilitation suppliers.
Specifically, the application narrative should describe a research methodology that will achieve the following:
1. Develop & pilot test data collection methods
2. Demonstrate inter-rater reliability of data collection methods
3. Provide summary data regarding the time and associated tasks that comprise provision of wheeled mobility and seating equipment by AT suppliers.
4. Provide evidence supporting the validity of the data.

Anticipated award amount: $80,000 (including direct costs & indirect costs)

Eligible applicants
All applications must be submitted by fiscally responsible organizational entities in the name of the Principal Investigator. Entities should be nonprofit academic institutions, health-care providers, associations, and/or organizations. Each application must include appropriate endorsement of an official who is responsible for the administration of awarded funds (i.e., a Grant Administrator). Grantee institutions must be located in the U.S. or Canada. Qualifed PI’s are expected to have a terminal research degree (e.g., PhD).
Overhead costs
The Sam Schmidt Paralysis Foundation recognizes that an academic institution to which a grant is made will incur additional operating costs while the program is in effect. In such cases, the SSPF will allow the institution a budget item for Indirect Costs (F&A charges) limited to 10% of the Total Direct Costs.
Letters of Support
Proposals for projects involving institutions or agencies in addition to the grantee (e.g., affiliated clinical facilities, laboratories to conduct work under a contract, etc.) must include a letter signed by a responsible official of the cooperating facility acknowledging its role in the project and supplying such assurances as may cover any aspect of the research to be conducted in the facility.

Review Process
All submissions are considered confidential. Applications will be reviewed by SSPF. Applications will be judged on scientific merit and overall quality of the research team.
Announcement of Award
Announcement of grant decisions will be made no later than March 2, 2009.
Grant Start Date
The successful applicant will be sent a Grant Acknowledgment Form to complete and return to the Foundation office. The grant will be awarded for a 12-month period, beginning no earlier than March 9, 2009 and no later than May 9, 2009.
Human Subjects Protection
All applicants must comply with rules and regulations published by the National Institutes of Health (NIH) on certifications and assurances regarding use of human subjects. Studies must ensure that informed consent is obtained from the participants. Approval documents from the appropriate Institutional Review Board (IRB) or committee must be specific to the proposed investigation.
Payment & Reporting Schedule
Payments will be made to the grantee institution in two installments:
The first payment of 75% of the total award is paid to the grantee upon the Foundation’s receipt of the original signed Grant Acknowledgement Form.
The second payment of 25% of the total award is paid to the grantee upon the Foundation’s receipt and approval of the grantee’s 3-month progress report, which is due 2 weeks after completion of the first 3 months of the 12-month project period.
The Final Expenditure and Progress Reports are due within 60 days of the completion of the 12-month project period. The Principal Investigator and Grant Administrator must sign all grant reports.
EXTENSIONS AND AMENDMENTS
Any proposed changes to the agreed-upon terms of the grant -- including grant period, personnel, budget, and location -- must be made in writing.
Such written requests must be signed by both the Principal Investigator and the Grant Administrator, and should provide a detailed rationale for the change. Agreement to an extension or amendment request must be acknowledged in writing by the Foundation to be considered binding.
The total amount of the grant cannot be increased; however, you may make minor changes within categories of your approved budget.
OWNERSHIP OF EQUIPMENT
All allowable purchases of apparatus and materials with SSPF grant funds will be the property of the grantee institution and are expected to be retained for use in related research activities.
PUBLICITY
All awarded projects are required to prominently acknowledge Foundation support in conference presentations, products, research publications, and other dissemination activities. In crediting the SSPF as a funding source, the following acknowledgment language must be used: “Supported by a grant from the Sam Schmidt Paralysis Foundation”. Additional co-funding sources must be credited in a similar statement.
